Waiver Forms for Group Trips
Parents of all students must complete the "Informed Consent form for Minors" and it must be given to your trip leader/teacher prior to boarding, or you will be left on the dock!
The "Waiver form for Adults" must be completed by all Adult Trip Leaders and Teachers.
