Spring Break Waiver Form

Each trainee must submit a completed Spring Break waiver or informed consent form (see below) to the SALTS office by fax (250-383-7781) or mail (#2 - 203 Harbour Road, Victoria BC, V9A 3S2) prior to boarding the Spring Break trip.  A trainee without a completed waiver or informed consent form will not be able to board the vessel.
